Billing Information

Clin-Path Associates, P.C. is contracted to provide pathology services at a variety of hospitals and facilities around the Metropolitan Phoenix area. These contractual arrangements require that the pathology group bill under different names and tax identification numbers in order to assure that payors and patients are billed correctly and that appropriate contractual allowances are provided when applicable. The following table represents the mechanism by which Clin-Path Associates, P.C. will bill patients depending on the location at which professional services were rendered:
